Fundamental to our mission at Shady Grove Fertility is to make fertility treatment more affordable and accessible to more people. In many ways, the number of physicians, office locations, financial programs, and various treatment options we offer helps to accomplish this goal. But our mission doesn’t stop at the door of SGF. Our mission is to create increased access for all who face infertility, which is why over the past 25 years, SGF physicians and, proudly, our patients, have been constantly involved in helping improve coverage and access to fertility care all over the nation. Sometimes, this change or increased access to care starts on Capitol Hill.

Listen Up, Lawmakers!

To make a change at the Federal level takes time and persistence, which is why SGF greatly values the individuals and couples who have bravely stepped forward to share their stories, our physicians who continue to fight for our patients, and our partner, RESOLVE: the National Infertility Association, who never ceases to stop fighting for all who might face infertility.

RESOLVE advocates to Federal officials on behalf of couples and individuals to make fertility care more accessible. But it’s not just RESOLVE who has a voice when it comes to making a change with the government, it’s you and it’s us.

SGF Physicians Make a Difference

When it comes to making a difference, many of our physicians have made it their personal goal to improve access to care. For example, just a month ago, Drs. Gilbert Mottla and Stephanie Beall of our Maryland offices advocated for fertility preservation prior to cancer treatment coverage to be included in the Maryland Mandate. Both testified at the local and state levels to help legislators understand how the additional stress and costs leave many patients with a very difficult choice: pay for cancer treatment or risk losing the ability to have a baby in the future.

In January 2016, Dr. Beall testified at the Senate to have the use of donor sperm for heterosexual couples to be included in the Maryland Mandate. She spoke on behalf of patients who would benefit from and would need assistance of donor sperm to have a baby. Fortunately, her testimony, along with fervent advocacy work, made a difference and in May 2016, it was added to the MD Mandate.

Remember the story of SGF patients, Kevin and Lauren Jaye? This couple faced fertility issues after Kevin returned from service disabled, suffering the loss of a leg and permanent reproductive injuries, after a roadside bomb in Afghanistan exploded. The Jayes and their families made it their mission to reverse the two-decade IVF ban that left wounded veterans without the fertility benefits they needed to have a family.

The Jayes, a former patient of Dr. Jason Bromer, of the SGF Frederick office, shared their story on CBS to raise public awareness and later attended Advocacy Day in 2016 to make Congress listen and hear their story. Their efforts helped win a small victory and funds within the VA are currently able to be used for fertility treatment or adoption services.

How can you make a difference?

  1. Attend Advocacy Day

    Over the past several years Shady Grove Fertility physicians, staff, and patients have joined RESOLVE to speak with legislators on Advocacy Day. During this daylong event, women and men living with infertility come together in Washington, D.C. to talk to members of Congress about issues important to our community. This event ensures that you have a chance for your voice to be heard. In preparation for the event RESOLVE provides registrants training and information needed for this empowering day. Advocacy Day is a great opportunity to meet RESOLVE leaders and others from the infertility community who want to make a difference.

  2. Write a Letter to Your Representative
    Can’t make it to D.C. for Advocacy Day? That’s ok! It only takes three letters to get the attention of the average representative and it can take as little or as much time as you have to put everything together. If you want to make sure your voice is heard, this form letter can be quickly printed, signed, and mailed off to D.C. If you have a little more time, the most powerful letters include a personal testimonial about your specific experience with infertility. Sharing your story is a great way to help representatives understand what their constituents need— insurance coverage through state mandates is a great example of legislation related to fertility care. Last year, hundreds of Shady Grove Fertility patients wrote letters to their representatives, making a strong impression among local leaders.

National Infertility Awareness Week® (NIAW) takes place this week (April 23-29, 2017), a yearly initiative founded by RESOLVE: The National Infertility Association, to encourage people to act as their own infertility advocates and to educate legislators and the public about how infertility impacts one in eight couples within our community.

Since establishing a partnership with RESOLVE, Shady Grove Fertility has helped to raise not only awareness about infertility, but tens of thousands of dollars in support through annual events that advance RESOLVE’s programming, such as support groups, educational content, public awareness initiatives, and advocacy to ensure that family building options are available to everyone.

#LISTENUP – 2017 National Infertility Awareness Week

The message behind this year’s theme, “Listen Up!” is intended to empower the community to raise awareness to our lawmakers, insurance providers, and employers by educating them about the prevalence of infertility. Together, Shady Grove Fertility and RESOLVE are asking those affected by infertility to educate their community about the importance of being proactive about reproductive health and to know the guidelines for when it’s time to see a fertility specialist.

NIAW is only one of the many ways throughout the year in which Shady Grove Fertility spreads awareness and supports the infertility community. On May 18, 2017, Shady Grove Fertility physicians and staff will let their voices be heard at Advocacy Day, a RESOLVE event, sponsored by Shady Grove Fertility, and in partnership with the American Society for Reproductive Medicine (ASRM), where the infertility community will come together to speak with members of Congress about topics that advance access to family building options and provide financial relief to those in need. This event is an opportunity to let voices be heard and connect with influencers in the fertility community.

Be Your Own Advocate

With one in eight couples experiencing infertility, with the number one predictor of infertility being the woman’s age, it is important for the community to come together and present a unified front, spreading awareness and letting couples know when it’s time to see a fertility specialist—given the importance of early intervention and how predominantly maternal age affects pregnancy outcomes. Shady Grove Fertility physicians typically recommend for a couple in which the woman is younger than 35, has normal cycles, and no risk factors for infertility, a year of trying to get pregnant without success is enough to warrant special attention. However, there are most definitely cases in which the 1-year standard isn’t the best course of action. Generally, when the woman is 35 to 39, a fertility consultation (to make sure there are no infertility-causing conditions besides age) should be considered after 6 months of unprotected intercourse, or after 3 months (preferably immediately) for women who are 40 or older.

In addition to age, other factors have such an impact on a woman’s fertility that a fertility workup is necessary, including:

  • Irregular or absent menstrual period
  • Genetic disorders
  • Two or more miscarriages
  • History of sexually transmitted infections, or STIs (formerly known as sexually transmitted diseases, STDs)
  • Endometriosis
  • Polycystic ovary syndrome (PCOS)
  • Family history or previous diagnosis of premature menopause
  • Male factor infertility
  • Endocrine (hormonal) disorders such as thyroid disease

Ask Your Employer to Provide Insurance Coverage for Fertility Treatment Health insurance provides reassurance that an individual will not have to bear the entire burden of his/her health care expenses, but when it comes to infertility, patients without insurance bear responsibility of covering the costs of treatment. With more than 7 million Americans faced with infertility, no one expects that their insurance company will deny them coverage for this medical condition or that employers will not offer infertility services as part of their insurance packages. Because of this situation, we are seeing a major road block for people who are not accessing fertility care because of a lack of insurance coverage. In fact, in a recent SGF survey, more than 95 percent of respondents said that finances stood as the #1 barrier to treatment. So why don’t more employers offer infertility benefits, and what can you do to affect change? It starts with speaking up …

What You Can Do to Advocate for Infertility Insurance Coverage within Your Company

The approach for advocating to your employer may vary based on the size and structure of your organization. For example, in a large company with over 5,000 employees you will likely need to submit a formal request through a human resources specialist at your organization. However, in a smaller organization, with 10 employees, you might be able to sit down and have a more intimate one-on-one conversation with your manager or the human resources point of contact for your employer.

Regardless of the approach, be prepared with supporting details for your request. Make it clear to your employer what you are asking for and what you perceive the impacts of that request will be for you, your colleagues, and the organization as a whole.

More women and men are being afflicted by the physical and emotional hardships of a disease that continues to be inappropriately viewed as “elective.” If you have been diagnosed with infertility and your employer does not currently provide infertility treatment coverage, put a formal plan in place to implement change.

Steps to Ask Your Employer for Coverage

1). Ask for change.

If your family building options are not covered through your employer’s healthcare plan or other employee benefits, why not ask for that change? Employers welcome input from their employers on the type of benefits that pertain to them. In understanding the needs of their employees, employers can make better, more educated decisions in the types of plans they select. The first step is asking for change and starting the conversation. By raising awareness about this gap in coverage, you’re bringing to the forefront an issue that likely impacts other people in the organization.

2). Write a letter to your employer.

RESOLVE: The National Infertility Association offers a sample letter for you to use that can be sent to your HR department or benefits manager. While the template provides an outline for key points to highlight, it’s best to personalize and customize it to you and your employer. Your voice is powerful and by adding your own personal touches it will provide your HR department with a full picture of the situation and why the need for change is important for you, other employees, and the future of the company.

3). Advocate for your rights.

RESOLVE, the only patient advocacy group in the U.S. that fights for the rights of women and men who are infertile, has seen an increasing need to bring together the community to ensure that our voices are heard. Advocacy Day, which takes place on Thursday, May 18, 2017, is a RESOLVE event where the infertility community comes together in Washington, D.C. to talk to Members of Congress about important issues, like increased access to family building options and financial relief to make treatment accessible and affordable for all.

When communicating with employers, insurers, and legislators, it is critical to share facts that support your argument. The more supporting information that is shared will allow your employer to make an informed decision about taking the necessary steps to provide insurance coverage for their employees.

It is up to all of us to change the perception of how others view infertility by educating the community and finding a resolution to the millions of Americans who are impacted. At Shady Grove Fertility, we know that staying on the cutting edge of fertility research is the best way to provide you the best quality treatment available. Therefore, we make continual study and practice refinement among our top priorities.

“But not every new idea or new technique that you see on television or read about in a medical journal is a practical solution for couples trying to conceive a baby,” reminds Shady Grove Fertility Medical Director, Eric A. Widra, M.D. With new fertility innovations taking place all the time, how do you know what’s really available to you—or what will be available to you soon?

Knowing which options are viable and those that need further research can help you to determine your best course of action. Here’s your guide to what you should watch in fertility innovation and what’s not worth your time.

Watch: Preimplantation Genetic Screening (PGS)

Preimplantation genetic screening (PGS) is the process of assessing embryo quality prior to implantation. While not a new practice, “people are increasingly using PGS,” says Dr. Widra. He continues, “Because of this screening, we’re no longer putting abnormal embryos into the uterus and can therefore increase pregnancy success rates.”

Dr. Widra continues, “Preimplantation genetic screening often results in a reduction in the pool of embryos from which a couple has to select. While this can be seen as a disadvantage, it is mitigated by the fact that the embryos that remain, while fewer in number, have a greater chance of successful implantation and eventual live birth.”

This procedure is similar to preimplantation genetic diagnosis (PGD). In PGS, however, fertility specialists assess only the number of chromosomes in the embryo. PGD, on the other hand, allows for the identification of specific genetic diseases or chromosomal abnormalities, including translocations.

Not: Sperm Generation

In 2016, researchers from the National Institute of Child Health and Human Development (NICHD) explored the feasibility of sperm development. While their findings seem promising—particularly for the approximately 30 percent of couples experiencing male-factor infertility, NICHD’s research has proven successful only in laboratory settings, not in real-world application.

“While it would certainly be beneficial to artificially produce sperm outside of the body, this technique won’t be coming to market soon and is not yet a dependable solution for patients seeking solutions for male infertility,” advises Dr. Widra.

Watch: Mini-IVF

Though highly effective, traditional in-vitro fertilization (IVF) requires expensive medications and a series of injections. Mini-IVF, or low-dose IVF, is an alternative that results in reduced costs and fewer injections.

“Because a woman is given less medication when using mini-IVF, she will produce fewer eggs per IVF cycle for extraction. For this reason, at Shady Grove Fertility, we don’t recommend mini-IVF as a front-line therapy for most as it is only truly beneficial for a select group of women,” adds Dr. Widra.

“We do think that there’s a role for Mini-IVF for women who are low responders, however,” says Dr. Widra. “Many naturally low responding women, or those who mature only a small number of eggs, will produce the same number of eggs no matter the dose of medication. Because the medication quantity makes no apparent difference in these women, the lower dose, which is part of the mini-IVF treatment, may be  more appropriate,” says Dr. Widra.

Not: Stimulated Egg Production or Improvement

Egg quality can be an impediment standing between many couples and their dream of parenthood. Traditionally, the only solution for these women has been to seek an egg donor. Some reproductive medicine specialists have been working to create another option, dedicating their time to exploring potential methods of either inducing egg production in older women or improving the quality of aging eggs.

“Unfortunately, there has yet to be any positive findings to suggest that either is possible. Like medically-induced sperm production, most fertility experts believe that egg production or improvement as viable fertility treatment is far from practical application,” says Dr. Widra.

Watch: Three-Parent Embryos

“Through the use of three-parent embryo techniques, clinicians have been able to help women with mitochondrial diseases reproduce without worry. Doctors can fortify the women’s eggs with healthy mitochondria and, in doing so, prevent mothers from passing their potentially debilitating diseases on to their children,” adds Dr. Widra.

“While life-changing for those dealing with mitochondrial diseases, these diseases are quite rare, so the application of this technique is relatively limited,” Dr. Widra advises.

Some researchers have theorized that these techniques could also be used to improve the quality of older eggs, though this theory hasn’t been proven true. Additionally, the FDA does not support the use of this procedure for this purpose.

Not: In-Vitro Maturation (IVM)

Through IVM, doctors extract immature eggs from a woman’s ovaries and create an environment outside of the body in which the eggs can develop to maturity.

Gathering immature eggs departs from typical egg retrieval procedures. Generally, women are given medication that encourages eggs to mature in the body. The mature eggs are then retrieved and used for embryo development within a fertility center.

“Because medication can induce egg maturation within the body, however, this particular piece of reproductive technology is largely unnecessary. Additionally, while some research suggests that IVM is possible, many studies employ medications to stimulate the eggs to mature before removal. The use of these medications hampers the validity of the studies,” advises Dr. Widra.

“Proponents argue that IVM could be a cost-saving alternative to IVF, but we consider those cost savings to be minimal as the money saved on maturation-inducing medication is instead spent on lab procedures. Additionally, IVM is a cumbersome procedure for both women and fertility doctors,” adds Dr. Widra.

“Given advancements within the fertility community, inducing egg maturation within a woman’s ovaries is not particularly difficult. As a result, IVM, while theoretically possible, is more of a regression than a step into the future,” says Dr. Widra.

Watch: Fertility Preservation

To the benefit of women everywhere, it is now possible to take steps to preserve fertility, a process commonly known as egg freezing.

Women can undergo fertility preservation for elective reasons. Women who are aging but haven’t yet found a partner or are busy with their careers and wish to start their families later commonly take advantage of fertility preservation options.

Fertility preservation techniques are also increasingly used with women who face diseases that threaten their natural fertility. For example, women with cancer can preserve the possibility of a child despite cancer treatments, such as radiation or chemotherapy, that may decimate their fertility.

Presently, doctors can engage in fertility preservation in a number of ways. Most commonly, doctors stimulate the production of eggs as they would if they were going to perform IVF, and then freeze them.

“Recent medical innovations have also made it possible to harvest ovarian tissue as a means of collecting eggs. Because this procedure is more invasive and the stimulation of eggs has become quite easy, this technique is most often reserved for young or prepubescent girls who risk losing their fertility as a result of cancer or another medical condition. By gathering this tissue, doctors can use IVF treatment to allow young women facing cancer to become mothers in the future,” adds Dr. Widra.

“In the coming years, we expect the application of these techniques to increase. The success rates continue to climb as clinicians do more research and improve their skills and understanding of human reproductions,” says Dr. Widra.

“While the medical community has yet to refine techniques to the point where we can help every couple experiencing infertility successfully conceive, scientific advancements and medical technology continue to bring us much closer to this goal. As research continues into 2017 and beyond, we expect the frequency of success of fertility procedures in general, and the birth rate in particular, to continue to rise,” Dr. Widra adds.

We are proud to announce the grand opening of our brand new, state-of-the-art center and laboratory in our headquarters location in Rockville, MD. The new expansive space totals nearly 51,155 square feet across two floors, and is home to physician offices for patient consultations as well as dedicated endocrine and andrology laboratories for procedures such as intrauterine insemination (IUI), sperm preparation, and patient monitoring. The fifth floor space is home to a massive state-of-the-art embryology laboratory—the largest freestanding and arguably one of the most sophisticated in vitro fertilization (IVF) labs in the United States. The 5th floor also includes a spacious ambulatory surgery center with 12 recovery bays, six embryo transfer rooms, and two operating rooms.

New Shady Grove Fertility Rockville, MD Location Now the Largest Freestanding IVF Lab

The Rockville office laboratory is accredited by the Joint Commission and the College of American Pathologists (CAP) operating under the distinct leadership of nationally and internationally known Jim Graham, laboratories director, and Michael J. Tucker, BSc Comb Hons, Ph.D., FIBiol, HCLD, director of Shady Grove Fertility’s IVF and Embryology Laboratories.

The embryology laboratory is equipped with the very latest technology including anti-shake tables, special ventilation, and inverted microscopes to evaluate embryo growth, development, and quality, to name a few. The highly skilled embryologists are specially trained to carefully handle patients’ sperm, eggs, and embryos and verify the identification of each specimen to ensure quality control and quality assurance, which is of utmost importance to the practice.

“The expansion of our new state-of-the-art clinical facility is attributed to the remarkable advancements we’ve made in our embryology lab and treatment protocols. In 2015, Shady Grove Fertility’s flagship Rockville, MD laboratory alone performed over 4,100 IVF and nearly 2,050 frozen embryo transfers (FET) treatment cycles—the highest number of IVF Program cycles ever performed at the practice in a single year. This is in addition to the 1,500 IVF cycles and nearly 700 FET cycles performed at the center’s two other state-of-the-art laboratories in Chesterbrook, PA and Towson, MD.
