Fertility Fact: Infertility affects over 7,000,000 women in the United States – up to 13% of female infertility may be caused by smoking – that is over 900,000 cases of infertility!

Smoking Can Delay Conception

Approximately 30% of all women in their reproductive years smoke cigarettes. Studies show that women who smoke experience a delay in conception, up to a 54% higher chance that conception will take a year or longer compared to nonsmokers. The delay in conception correlates with daily quantity of cigarettes smokes.  The more cigarettes smoked, the more chemicals enter your body, increasing the rate of follicular depletion and reducing the estrogen in your body.  This is also why smokers reach menopause several years earlier than nonsmokers.

Smoking & Miscarriage

The label says it right on the box – smoking and pregnancy don’t mix. Studies have shown chromosomal and DNA damage from smoke exposure increasing birth defects and miscarriages. In fact, one study done for female smokers 14-39 years of age, showed smoking accounted for 16% of miscarriages.

Cancer and heart disease are well known risk factors for smoking… but only 22% recognize infertility as a health risk for smokers.

Even with infertility treatment, smoking can make conception more challenging. Smokers may need increased medication for ovarian stimulation, may produce a lower number of eggs for retrieval, have increased number of cancelled cycles, and have more cycles with failed fertilization compared to non-smokers.
Like many things – quantity matters. Women who smoked briefly in their lives might experience some decreased success with infertility treatment, and women who smoke larger quantities over many years showed the greatest risk of unsuccessful treatment. The final result is smokers as a group require nearly twice the number of IVF cycles as nonsmokers to conceive.

Need to Quit Smoking?

In addition to the reproductive issues, the risk of cancer is the single largest reason to stop smoking.  Shady Grove Fertility physician, Joseph Doyle, MD suggests that “One of the hardest conversations to have is telling a patient who smokes that their chance of success during IVF is half of what it could be. “

Shady Grove Fertility is proud to welcome a new physician to the team – Kimberly S.  Moon, MD. Dr. Kimberly Moon will be seeing patients in our Washington, DC office on K St. starting next week. You might have seen her during her orientation and training in the Rockville and DC offices but here’s a little more information about Dr. Moon.

Dr. Kimberly Moon’s Road to Shady Grove Fertility

Dr. Kimberly Moon took a different path to becoming a physician than most. After completing her undergrad degree in economics at Boston College, she traveled to New York City where she took a job in the fashion industry. A “Jane-of-all-trades” as she liked to call herself, Dr. Moon was working with buyers and handling finances for Kenneth Cole.  After a few years, she found that she was not feeling challenged enough and was looking for a career that was more fulfilling.  She decided to go back to school. She took the necessary science classes and started at the Georgetown University School of Medicine.

A focus on women’s health was an obvious match for Dr. Moon. “I’ve always liked OB/GYN, working and connecting with women.” Plus, she really enjoyed her endocrinology classes which made her focus on Reproductive Endocrinology. What sealed the deal? Shady Grove Fertility’s Dr. Eric Widra.

Dr. Widra was giving a lecture at Georgetown on Reproductive Endocrinology and Infertility during Dr. Moon’s residency that convinced Dr. Moon that her specialty should be Reproductive Endocrinology.  Funny coincidence, Dr. Jason Bromer (now at Shady Grove Fertility’s Frederick, MD office), who was then Dr. Moon’s chief resident, was also in that lecture hall becoming just as inspired to pursue infertility medicine!

Dr. Moon and Widra stayed in touch as she went on to complete her Reproductive Endocrinology and Infertility fellowship at the National Institutes of Health. She has even helped with several research projects Shady Grove Fertility has conducted over the past few years, solidifying her as a perfect match for SGF.

Dr. Moon is energized to begin seeing patients next week at the K St. location in Washington, DC. “I am excited to connect with other physicians and to help patients overcome infertility.”

Dr. Moon is a straight-shooter who knows that fertility patients do their research! And her patient philosophy? Honesty is the best policy. “I believe in being honest and having open communication. I want to explain to patients why something is happening versus just telling them what to do.”

When asked what is the one tip she would share with a patient? “Don’t label yourself or limit your possibilities. With so  many advances in treatment, after we  do the fertility work-up and get to know each other, we can help nearly everyone conceive.”

Dr. Kimberly Moon grew up in Annapolis, MD and has kept to her local roots. Currently in DC, she still goes out to Annapolis on the weekends where she and her husband enjoy racing sailboats.

And her lifelong goal? Travelling to all the continents. She recently travelled to Australia and just has Asia and Antarctica left to check off the list!

Fertility Fact: Smoking decreases sperm quality leading to male infertility.

Smoking and Male Infertility

Smoking cigarettes has substantial harmful effects on fertility.  Couples undergoing IVF who smoke have approximately HALF the rate of success per cycle compared to non-smoking couples.  In addition, sperm gene damage can occur from direct binding of the toxins in cigarette smoke to DNA.

Smoking cigarettes can cause a decrease in the three main factors that determine sperm quality:

  1. Sperm Count – an average 22% decrease in men who smoke regularly
  2. Morphology – the shape of the sperm
  3. Motility – the ability of the sperm to swim forward and penetrate an egg

Smoking and Erectile Dysfunction

Smoking has also been linked to erectile dysfunction and impotence in men of all ages. Why? Blood flow.

  1. Carbon monoxide from cigarettes can increase your blood pressure and heart rate. The result is a strain on the heart that then makes the heart unable to pump enough blood to maintain an erection.
  2. Smoking may also result in fat deposits in blood vessels that restrict blood flow to all organs.
  3. Smoking may lead to hardening of the arteries – cardiovascular disease – which can also restrict blood flow.

The Good News

Studies have shown that damage is not necessarily permanent and may vary by amount and length of time you have been smoking.  Studies have shown a benefit from quitting smoking by at least 2-3 months, and according to Shady Grove Fertility physician, Dr. Joseph Doyle, “Much of the reduction in fertility rates can be completely reversed within a year of stopping.”

What is Complementary and Alternative Medicine (CAM) and how can it help patients during treatment?

Last week, I had the privilege of presenting about this very topic as part of the post graduate program faculty for the American Society for Reproductive Medicine Conference (ASRM) in San Diego, CA. The participants included physicians and practice administrators from all over the U.S. and abroad. I have to tell you, before I began my research to prepare for the talk, I was as unsure as you probably are about the benefits of complementary or alternative medicine, and maybe even a little skeptical. After extensive research my perspective has changed and I am now a firm believer in the potential value holistic services can provide to patients undergoing fertility treatment.

Top 4 Lessons Learned

  1. 40% of americans use alternative medicineThe National Institutes of Health created the Center for Complementary and Alternative Medicine to establish scientific rigor around studies of effectiveness. The most recent research is astonishing – 40% of Americans use complementary or alternative medicine care. http://nccam.nih.gov/
    Take Away: More Americans are using Alternative Medicine for wellness AND treatment.
  2. In 2007, nine leading U.S. Hospitals, including Duke and University of Maryland, embarked on a new venture to change the way the medical community thinks about health and healing. They established the nation’s first practice-based research network in integrative medicine. http://www.bravewell.org/
    Take away: Alternative medicine is being fully integrated into the traditional healthcare models of respected institutions.
  3. A review of the scientific literature shows mixed results when it comes to definitively proving Alternative Medicine techniques improves pregnancy outcomes. However, there is very good evidence demonstrating a reduction of stress levels can positively impact outcomes.
    Take Away: There are many ways patients can reduce stress with complementary services such as yoga and acupuncture, just a couple of examples. Exercise, meditation, prayer, support groups, and even laughter can work as well. It is a matter of personal preference.
  4. Shady Grove Fertility, Pulling Down the Moon, RESOLVE, Attain Fertility and the Domar Center conducted a Fertility Patient Usage and Attitudes survey about Complementary Medicine. 800 patients responded and the results were eye-opening. We learned that many of our patients are already using alternative medicine and that 90% believed it had a positive impact on their stress level, ability to stay in treatment longer and/or pregnancy outcomes. Below are a few of the key points from the survey.
    Take Away: By offering these services through approved providers, Shady Grove Fertility can help patients navigate their fertility journey with more balance.

     

    1. 2 out of 3 have used alternative medicine to improve fertility2 out of 3 survey respondents have used complementary or alternative medicine specifically to improve fertility
    2. 33% believe alternative medicine helped them conceive
    3. 69% thought alternative medicine helped them reduce stress & relax
    4. 49% say alternative medicine gave them a sense of control
    5. Just 10% didn’t think alternative medicine provided any benefits
    6. 64% of patients self-referred to a alternative medicine provider (21% from friend, 15% from a physician or nurse)

One of the questions I asked of the post graduate participants was “How many of you are offering Complementary of Alternative care through your fertility center?” Most of the audience raised their hands. The ensuing discussion focused on the best way to integrate these services and how to make them affordable for more patients. The West is opening to the East and as a result more patients will benefit.

The Washington Post and Super Doctors have announced their 2012 list – and many of Shady Grove Fertility’s physicians have been recognized!
