Polycystic Ovary Syndrome (PCOS)

I was 18 years old when I learned I had polycystic ovary syndrome (PCOS). I wasn’t really considering children then, so it didn’t matter to me-nor was it really explained to me what it meant and how it would affect me later in life. I met my husband in 2012. We started dating in 2013 and married in 2015. Our story is a little different. We met Dr. Kallen when he was at another fertility clinic and then knew we had to follow him to Shady Grove Fertility.

Beginning our path to parenthood

I was seeing my OB/GYN and doing Clomid, and we were having no luck. My nurse at the time, Sarah, told me about another fertility clinic. We made an appointment in August of 2015 and were introduced to Dr. Kallen. Like me, he shoots straight from the hip, he’s honest, up front and doesn’t pretend with you. I absolutely LOVED that about him.

We did a few intrauterine inseminations (IUIs) with no luck. In January 2016, we did in vitro fertilization (IVF). We had two transfers which both ended in miscarriage. We were still a young couple, so we decided to take a break. We came back in October, transferred our final embryo, and it didn’t implant. I was ready to quit. But after a heart-to-heart conversation with Dr. Kallen, he convinced me to give it one more try with an IUI, and I agreed.

On a November afternoon, Dr. Kallen called me at work to tell me the IUI worked, and we were pregnant. The next 10 weeks went by quickly and I ‘graduated’ to my regular OB/GYN. In July of 2017, I gave birth to a healthy, 7lb 8oz baby boy, whom we affectionately name Caleb. If it hadn’t been for Dr. Kallen’s convincing, Caleb wouldn’t be here.

Finding fertility care at SGF

In 2019 we found Dr. Kallen had moved to Shady Grove Fertility. I was ready to give it another go and knew we would want Dr. Kallen on our team. We tried a few IUIs, but they didn’t work. I was also in college, working full time, and raising an almost 2-year-old. I think Dr. Kallen knew I wasn’t there mentally. He was more than receptive when I told him I needed a break.

When we came back in 2021, Dr. Kallen was there, ready and waiting! We did an IUI or two, and I did get pregnant, but it was in my tubes, and I think we all got scared. Because of my age (41), Dr. Kallen sat me down and told me that it wasn’t safe to continue with the IUIs. He recommended we move forward with IVF. We had a retrieval in October of 2022, did all the appropriate testing, and Dr. Kallen transferred our healthiest embryo on December 17, 2022. In August of 2023, we welcomed Luca, our second healthy baby boy at 6lbs and 4oz.

If it hadn’t been for Dr. Kallen’s commitment and dedication, we wouldn’t be parents. Dr. Kallen kept me motivated throughout the process. He gave me grace when I needed it, he gave me time when I needed it and most importantly, he gave me hope — but it came with no promises.

Holding on to hope

Infertility is stressful. IVF and the hormones and the outcomes are hard to deal with. You have to remain hopeful no matter what. Dr. Kallen gave that to us. That hope has given us two beautiful boys who are now 7 and 1.

I am eternally grateful to Dr. Kallen for his determination and because he didn’t give up on me when I was ready to give up on myself.

I’d tell future patients: Do your research, keep an open mind, don’t give yourself false hope, and don’t settle for a doctor that gives it to you either. Find a support group for you and your partner. Keep the communication open with you and your partner and PRAY!

My life had been a battle since I started my cycle at age 13. Countless doctor visits. None of which could tell me what was wrong. Why did I only have a cycle every 3-6 months? I was continuously told it would work itself out, and had birth control pill after birth control pill shoved down my throat. I eventually got tired of taking the pill, so I stopped. I went 18 months without a cycle and was then given medication to induce it. This cycle of no period and drugs to induce it went on for years. At one point, around 20 years old, I was told I should just go ahead and have a hysterectomy because I couldn’t have children anyways. This was devastating. To have multiple doctors tell me a family was never in my future without actually performing any worthwhile tests.

After 10 years together, my now husband and I got married. I had been off the pill for all 10 years and no sign of having a cycle. No positive pregnancy tests. I decided there had to be more I could do to try and start a family together. My cousin mentioned Shady Grove Fertility, so I decided to give it a try. Worst case, they’d tell me what I’ve been told my whole life, right?

And so, the process began. From my very first video appointment, I didn’t feel dismissed. I felt heard, and that was refreshing. They immediately ordered all sorts of tests to get to the bottom of my issues. My husband got tested as well just to make sure any treatment for me was worthwhile and he didn’t need treatment as well. Lo and behold, they found I had polycystic ovary syndrome (PCOS).

I didn’t quite know what this meant for us, but they took their time and explained it all in a manner that was comprehensible. They started me on a regimen of medications to get my body to actually ovulate. We had to travel 4 hours round trip every 3 days to monitor my cycle, but the fact that everyone there was so welcoming and positive every time we went made it worth it. We started by trying timed intercourse, with no prevail. The staff was so reassuring even though we got a negative result. They didn’t let us feel like it was a lost cause. I spoke with my doctor and the financial educator at SGF and asked if it was possible that we skip the next two cycles of timed intercourse and go straight to intrauterine insemination (IUI). Luckily this was approved by insurance, and we gave it a go. Another round of medications and then the scheduled IUI. We were so incredibly lucky that it took on the first try! To see those two pink lines was a miracle. We now have a healthy, happy, three-week-old baby boy.

Shared joy

None of it would have been possible without the caring staff at SGF. I am forever grateful for them getting to the bottom of my fertility issues, treating them quickly, and showing compassion the entire way. They always made sure to check on my mental wellbeing at every appointment. The excitement throughout the office when we got our positive pregnancy test was so amazing. They shared our joy with us as if they were family. Infertility is a long, hard road, but SGF makes it a little more bearable. I want to give a huge thank you for making our dreams come true.

We knew we wanted to be parents, but we certainly didn’t expect it to be as difficult as it was to get there. We were married in a small, civil ceremony in June 2020, but coming from a Greek background and being raised in the South meant that we had always dreamed of a big wedding. While we were able to celebrate more in June 2021, the giant wedding never happened. However, we were so excited because we felt like it was finally time to start building our family.

Shortly after our wedding, we began the process of trying to build our family. I stopped my birth control, which I had been taking since I was 17 years old, and immediately started noticing some negatives. I had horrible symptoms of weight gain, acne, losing hair on my head while also experiencing hirsutism, and most glaringly, no periods after the first initial withdrawal bleed. I talked to my OBGYN about these issues, and she immediately diagnosed me with PCOS (polycystic ovary syndrome). She told us that she would be happy to help us by providing induction medication, but she also suggested we reach out to Shady Grove Fertility for further information as it would be likely that we would need some more help to grow our family. I began taking Letrozole to induce ovulation. After the third dose, we found one that worked for me. So, for 3 months, we tried timed intercourse with the Letrozole hoping for a positive. That positive never came. We knew it was time to reach out for more help, so we made a consultation appointment at SGF and were matched with Dr. Anish Shah. 

Dr. Shah was wonderful from the beginning. He took the time to explain everything to us in great detail and told us all the steps that we would take to discover the best course of action for us. As we began testing, we found out that in addition to my PCOS, my husband also had a low sperm count and mobility. Because of this, we also worked with Virginia Urology to determine the best course of action. It was decided that we would undergo in vitro fertilization IVF with intracytoplasmic sperm injection (ICSI). While we were excited to get started, we knew we had a long road ahead, so we braced ourselves. After going through the process, we ended up with two PGT-normal embryos, one boy and one girl. This was truly a rollercoaster because it initially looked like we would only have one healthy embryo, but our other one pulled through, and we are eternally grateful. 

March 28, 2024, came and it was the day — transfer day! We were so excited to transfer our sweet little embryo. For a month leading up to transfer day, I did everything to relax that you could think of, and I went to acupuncture, just for good measure. A few excruciatingly long days later and we got the call, we were pregnant! We couldn’t believe that our first transfer had worked, and we were so thankful to God and SGF for helping us get to that point. 

A sweet little boy after hardship

My pregnancy wasn’t an easy one. We did unfortunately suffer from the loss of a twin that had split from our embryo, which was extremely difficult. I also had a sub-chorionic hematoma, which was scary, but thankfully resolved on its own. Finally, my pregnancy ended in an emergency C-section due to severe preeclampsia and then a prolapsed cord. It was safe to say that our boy gave us a wild ride into the world, but he was healthy and born at 5lbs 1oz, and 17inches. Our sweet little boy, JB, wanted a November birthday instead of December!

Support and kindness every step of the way

I would tell future patients to not wait. My only regret is not going to SGF sooner. The support and kindness we have felt every step of the way made the extremely difficult process that much better. We are so thankful that God placed us in Dr. Shah’s hands and would recommend anyone to go to SGF for their infertility journey.

We always dreamed of having a family and thought it would happen quickly and easily. I would discuss family planning with my OB/GYN and was told repeatedly that I was “healthy and young and it’ll happen.” I left every appointment feeling discouraged.

Everyone always said, “It will happen if you stop stressing” but that was the most frustrating piece of “advice.” How can you not stress or worry about the unknown of whether you can start a family? We needed to get answers and weren’t going to give up.

At one point, I did test positive for pregnancy, but the next day my menstrual cycle began. Looking back, this was more than likely a miscarriage.

Finding fertility care at SGF

After a few years of trying, I went to a different OB/GYN who took me seriously and she suggested finding a fertility center to help. My doctor then started the process of running tests to see what the issue(s) may be. I went straight home to research fertility centers near me and found Shady Grove Fertility. I called the office that day and our journey started. Everyone should advocate for their own health and push if you truly think there is an issue. I wish I would have sooner.

We started the process with SGF and had a consult with Dr. Nguyen, she was so compassionate and really listened to our struggles. We put a plan in place and got started. We started to feel some relief and hope as we knew we were on the right path.

After my results came back, it was discovered that I had polycystic ovary syndrome (PCOS) as well as a uterine condition that could cause miscarriages and difficulty getting pregnant. These were both conditions that had gone undetected for years.

Making a personalized treatment plan

We proceeded with the in vitro fertilization (IVF) process, had an egg retrieval, and then ultimately did a frozen embryo transfer. To our relief, it was a success. However, a few weeks later, we suffered a devastating miscarriage. Dr. Nguyen was extremely compassionate as well as the entire staff at the Wexford location. This miscarriage was the worst time of our lives.

Dr. Nguyen suggested surgery to fix the uterine issues. We traveled from Pittsburgh to UMPC in Mechanicsburg for the surgery. The problem was more severe than we thought, and I underwent a second surgery two months later.

After healing from surgeries, we prepared for our second frozen embryo transfer.

Our chance again

After this transfer, I just started crying tears of happiness and of hope as I felt like this was our second chance. The staff who performed the transfer were so happy for us, gave us the biggest hugs and were rooting for us. It truly felt like all the years of trying, the years of grief, and all the shots, medications, and procedures just washed away.  

After bloodwork, it was confirmed that I was pregnant!  

We felt so incredibly excited but in the back of our minds we worried about our past miscarriages – which I’m sure many couples who’ve experienced pregnancy loss can relate. 

After a few weeks, we graduated from SGF in Wexford and the entire staff had a graduation ceremony for us. This brought us to tears as we had come to know everyone and just felt so supported by them. It truly felt surreal as we walked out of the door. My husband and I hugged and cried in the parking lot as we couldn’t believe this was really happening.  

When we graduated, our little baby was the size of a gummy bear and we nicknamed our little one, “baby bear.” 

We wanted to be surprised, so didn’t find out the gender ahead of time. It was fun guessing who our little one would be and fun to see our family and friends guess as well.  

At 39 weeks, our baby bear was measuring big and around 8 pounds, and so we were induced ahead of our due date.  

The joy of our lives

On April 10, 2024, our perfect son was born. Hearing my husband tell me, “It’s a boy” is a memory I will always cherish. Holding my son in my arms took away all of the pain that we endured over the years. We never felt love like that in our lives.  
 
We named our son Rogan – 8 pounds, 1 ounce, and 21 inches of just pure perfection. He is the joy of our lives, with big blue eyes, the sweetest smile, and the curliest hair (which is so wild, it can’t be tamed).  

All of the heartache, struggles, stress, were well worth it. We would do it all a million times over if it meant we could have our Rogan. He is the love of our life. We truly are blessed that he chose us to be his parents.  

I never knew how common infertility is before enduring it, and hope that those going through infertility keep their heads up, lean on others for support, and that their dreams come true. We couldn’t have done this without Shady Grove Fertility and are beyond thankful for our miracle baby, Rogan.

My husband Dominique and I are high school sweethearts, been together since 2009. We got married in 2019 and knew we wanted children. Our journey began in 2020 shortly after we were married. A few months went by, and I hadn’t become pregnant, but didn’t think much of it.

I talked with my regular OB/GYN who said we should do some testing. A few days later, my world was flipped upside down when I was diagnosed with polycystic ovary syndrome (PCOS) and was told it may be difficult to get pregnant. We tried Clomid and Letrozole with no luck and were referred to a fertility specialist.

Finding fertility care at SGF

This began our long journey to parenthood. We attempted multiple intrauterine inseminations (IUIs) with no success and the words in vitro fertilization (IVF) were starting to be the talk. I remember thinking how in the world can we do this?

Well life happened and then a pandemic and a few years went by. I transferred jobs and found out my insurance would cover IVF, so the planning began.

Everything was going great; the medications were working, and it was good news after good news.

It came time for transfer and the first one failed.

Cue the tears, but our faith in God remained. Let’s try again … fast forward and another big negative. At this point, I thought I would never be a mom.

More testing revealed I needed one more day of medications prior to transfer, so we tried again. Well .. another negative. We were heartbroken.

Adjusting our personalized treatment plan

My amazing doctor, Dr. Emily Evans-Hoeker, had no explanation but wasn’t giving up and suggested a new protocol with aspirin, a steroid, and an antibiotic prior to transfer. We agreed and prayed hard on it.

Fast forward to June 26th and I got my first positive! We were pregnant! Pregnancy went great and in February 2024, we had our miracle baby boy Donte.

I am so thankful for Dr. Evans-Hoeker and her staff who never gave up on us and encouraged us every step of the way. Our lives were forever changed because of them, and I’ll never have the words to thank them enough.

After dating for a while Dakota and I got married in 2015. We knew from early on in our relationship that family was very important to us, and we would love to grow our family.

Soon after we were married, we decided to start trying. The months turned into years, and we had no success. Our friends began having children, and they became our adopted nieces and nephews but still, we longed to have children of our own.

At the end of 2018, my OB/GYN diagnosed me with polycystic ovary syndrome (PCOS) and suggested Dakota and I begin our fertility testing together. After many appointments, testing, and labs we found out Dakota has a genetic defect causing us to have male factor infertility.

We immediately started exploring our options and scheduled an appointment with an adoption agency. As the days grew closer to the appointment, I came across a Shady Grove Fertility advertisement and felt it was a sign to not give up on the idea of pregnancy.

Feeling supported and hopeful at SGF

From the very first appointment with Dr. Matthew Connell, Dakota and I felt heard, understood, supported, and above all, hopeful! Soon after the appointment we chose our sperm donor and started our intrauterine insemination (IUI) cycle. Doing these treatments during the pandemic was not the easiest, in fact it was rather lonely. We would drive to the clinic together, but Dakota would have to stay in the car. The first IUI cycle failed, and so did the two we did after.

We felt so defeated. We decided we needed to take a little time to regroup and meet with Dr. Connell to determine what the next move was going to be. Together we decided in vitro fertilization (IVF) would be worth a try. Dakota and I both felt overwhelmed, so we took some time off treatments before jumping in.

In November 2023, we started our first IVF cycle. I was nervous about the shots and the retrieval. Both of which were not as bad as I anticipated. On January 19, 2024, in the middle of a snow and ice storm, we made our way to Towson for our first frozen embryo transfer (FET).

Two weeks later we got the most amazing phone call from Dr. Connell letting us know I was pregnant! After the fastest 9 months, on September 24, 2024, we welcomed our son, Carson William into this world!
