Not sure where to begin? Know Your Fertility is a simple first step, built by fertility physicians. For $99 per person: a reproductive health questionnaire and AMH testing or semen analysis, reviewed by our clinical care team including a reproductive endocrinologist, plus a personalized phone consultation with a senior fertility nurse and clear next steps.

Accepted insurance

At this time, SGF Carolinas is in network with Aetna, BCBS and Cigna insurances and our accepted fertility benefits including Progyny. All other appointments will require self-pay for visits and treatments.
The SGF care team is actively working with insurance companies and hope to allow for insurance coverage for applicable insurance plans in the upcoming year. SGF is proud to make fertility care accessible by offering the Shared Risk 100% Refund program for self-pay patients. Your dedicated financial counselor will discuss all pricing and financial programs with you after you meet with your physician.

SGF’s accepted fertility benefits

SGF is proud to make fertility treatment more affordable by accepting exclusive fertility benefits provided by employers/companies as well as third-party administrators. SGF patients who choose to use their benefits from the below list of providers will enjoy negotiated preferred service rates and a streamlined claims process.
Some fertility benefit administrators may require employees to enroll directly with their organization in order to be eligible for fertility treatment benefits. Please connect with your employer’s HR department for more information about available fertility benefits, or your SGF Financial Counselor to get started.

Directions

From interstate 75, take exit 213 for University Parkway, and head east. Continue for approximately 2 miles, then turn left onto Lakewood Ranch Blvd. Proceed north for about 1.5 miles; the destination will be on your right. If coming from the north via State Road 70, head east until you reach Lakewood Ranch Blvd. Turn right (south) onto Lakewood Ranch Blvd and continue for about 2.5 miles. The office will be on your left. We are situated within the Lakewood Ranch Medical Center campus with ample, free parking.
