Nutrition and lifestyle for sperm health

Sperm health responds to how you live, often more than people expect. Because sperm take time to mature, changes you make now show up over the following few months. Here is what the evidence supports.

Diet and nutrition

A balanced diet rich in fruits, vegetables, whole grains, lean protein, and healthy fats supports sperm health and overall reproductive wellness. Antioxidant-rich foods, such as berries, nuts, leafy greens, and tomatoes, may help protect sperm from oxidative damage. Stay hydrated with 1.5 to 2 liters of water daily, and limit caffeine to no more than one to two cups of coffee or the equivalent per day.

Movement

Moderate-intensity exercise for 30 to 45 minutes daily supports reproductive wellness. Moderate intensity means a slight increase in heart rate and a light sweat. If you do not currently exercise, a good starting point is low-impact activity such as swimming, walking, yoga, or Pilates.

Weight, movement, and heat

Moderate-intensity exercise for 30 to 45 minutes daily supports a healthy body weight and hormone balance, both of which benefit sperm production. Carrying excess weight can affect sperm production and testosterone levels, and even modest weight loss may improve sperm parameters. Avoid the opposite extreme, too, since excessive or extreme endurance training can work against you. Prolonged heat to the scrotum, including hot tubs, saunas, and laptops rested on the lap, can temporarily impair sperm production and is worth minimizing.

What to avoid

Cigarette smoking is associated with lower sperm concentration and motility and increased DNA fragmentation, so quitting is strongly recommended. Excessive alcohol can disrupt hormonal balance and reduce semen quality. Anabolic steroids, exogenous testosterone, cannabis, and opioids may suppress sperm production. If you use any of these, it is worth discussing with your care team.

Supplements

Antioxidant supplementation may modestly improve sperm quality by reducing oxidative stress. Supplements with evidence behind them include Coenzyme Q10 (CoQ10), Vitamin C, Vitamin E, and zinc, and a general multivitamin is a reasonable starting point. Folate may also benefit sperm DNA integrity and is worth considering when planning for conception.

Establishing care with a urologist or primary care provider is an important part of maintaining reproductive and overall health. Some sperm-related findings can be associated with underlying medical conditions that warrant further evaluation, so if you have not had a recent physical, this is a good time. Your care team can help guide next steps based on your results. If you do not already have a urologist or primary care provider, your health plan’s directory is a good place to start.
