What’s included

  • A reproductive health questionnaire developed by our physicians. This is the part that gives your test results important context and allows us to personalize your consult.
  • Your testing, an AMH blood draw or a semen analysis.
  • A full review of your questionnaire and results by your clinical care team.
  • A personalized consultation with a senior fertility nurse, who walks through what your results mean for your fertility or family-building goals.
  • Clear next steps, if your results suggest you may benefit from a full consultation with a reproductive endocrinologist. We can help you schedule, and your $99 is credited toward future services.

What it costs

$99 per person, a one-time cost for your testing and consultation. No insurance needed, this is a simple self-pay rate. HSA and FSA eligible. Your payment is credited toward future services if you decide to move forward at SGF.

Know Your Fertility is not a diagnostic evaluation and does not establish or rule out fertility or infertility. Getting started

How it works

1. Schedule

No referral or insurance required. Call 1-888-761-1967 to schedule your testing or request an appointment online.

2. Share

You’ll complete your reproductive health questionnaire, so our team can focus on the areas most relevant to you.

3. Test

Visit your SGF location for an AMH blood draw, or collect your semen analysis sample at home and drop it off. Learn how to prepare for a semen analysis. You’ll schedule your results consultation when you come in.

4. Consult

On a scheduled phone call, our team reviews your results with you and explains whether meeting with a reproductive endocrinologist is a recommended next step.

And if you’re a couple

You can each complete your own testing and questionnaire, then book your consultations back-to-back so your results are reviewed together. Know Your Fertility is $99 per person, or $198 total when you test alongside a partner.

Results + consultation

Your reproductive health questionnaire and test results are reviewed by your clinical care team, and discussed in a phone consultation. You’ll understand them in the context of your reproductive health and long-term family planning goals.

Testing for sperm-producing individuals

Semen analysis:  A semen analysis measures sperm health factors that help inform what may affect your ability to conceive. It is a point-in-time snapshot. In addition to total motile count, concentration, motility, and morphology, your full analysis reviews semen volume, vitality, and other markers.

Testing for egg-producing individuals

Anti-Müllerian hormone (AMH):  An AMH blood test is a marker of ovarian reserve (egg quantity). It does not indicate egg quality; age is the best predictor of that. Reviewed alongside your reproductive questionnaire, AMH helps you understand whether seeking additional evaluation makes sense. And if you later pursue fertility treatment, AMH helps your physician evaluate how you may respond and guide medication dosing.

Know Your Fertility does not diagnose infertility or any health condition, and it does not establish or rule out fertility. Your AMH or semen analysis results are a point-in-time snapshot, not a complete evaluation. AMH is a marker of ovarian reserve, a measure of egg quantity, and does not evaluate egg quality. Reviewed alongside your history, your results can help you understand your options and decide whether seeing a fertility specialist is a sensible next step.

You are not ready to start a family yet, but you would like to understand your timeline. A simple check tells you where you stand today and whether you need to meet with a fertility doctor, also known as a reproductive endocrinologist.

You would rather start with facts and a clear explanation than spend months wondering. Your results give you a picture of where things stand, and a more informed conversation with a specialist if you want one.

Understanding test results in the context of your history helps your SGF team build the most informed and personalized plan from day one.

What to expect

Frequently asked questions

With Know Your Fertility, our team reviews your reproductive questionnaire alongside your test results. A senior fertility nurse reviews your information and discusses whether meeting one of our fertility specialists is recommended.

A new patient consult is a longer clinical visit where you meet directly with a fertility physician, who reviews your full medical history and provides tailored recommendations and next steps based on your fertility goals. This may include additional evaluation and treatment recommendations.

You may opt to start with Know Your Fertility and move to a consult later if your results or goals point that way.

Neither. Know Your Fertility is an entry-point assessment – it gives you meaningful data about your reproductive health. It is not a diagnosis and does not initiate treatment. If your results suggest follow-up, our team will explain what a full clinical consultation would involve.

Available to individuals and couples ages 18–44 years who are not currently receiving care at SGF. Available up to two times per person, with a minimum of 12 months between visits.

Both. Individuals and couples are welcome. Sperm-producing patients can schedule independently – an egg-producing partner is not required. Each person pays $99 for their testing and consultation.

If presenting as a couple, your consult can be booked back to back for a complete picture.

Your senior fertility nurse will review your results with you during your phone consultation. If further evaluation is appropriate, they will discuss next steps – but there is no obligation to continue. You’re in charge of what comes next.

No referral is needed. You can schedule Know Your Fertility online or by phone at any participating Shady Grove Fertility location.

AMH (anti-Müllerian hormone) is a marker of ovarian reserve, or your remaining egg supply (egg quantity) at this point in time. It reflects how many eggs you have not the quality of those eggs, as egg quality is primarily influenced by age.

If you pursue fertility treatment, an AMH test also helps your physician evaluate how your ovaries may respond to medication and guide treatment planning. While AMH provides valuable information about ovarian reserve, it cannot predict whether or when you’ll conceive. A comprehensive evaluation with an SGF physician is the best next step for a complete picture of your fertility.
