The Importance of Fundraising

The Race for the Family is the Cade Foundation's largest annual fundraising event. The proceeds go to help fund their Family Building Grants, $10,000 grants to couples that are looking to pursue adoption or fertility treatment. Last year, in thanks to YOU, our sponsors and fundraisers, we raised over $65,000! We look forward to contributing even more this year.

About Cade Foundation

The Tinina Q. Cade Foundation, Inc. is a non-profit organization focused on helping families struggling with and OVERCOMING infertility. In 2005, at 55-years old, Tinina Q. Cade gave the ultimate sacrifice of love to her child. She carried and delivered her daughters triplet children. The Tinina Q. Cade Foundation strives to serve others struggling to have children with this same spirit of love, encouragement and support.

Win a Free IUI, IVF or Donor Egg Cycle Drawing Complete Official Rules

Eligibility. Drawing entry fee is $25. Entrants must be 18 years or older and U.S. residents. Up to five (5) entries per household. Employees of Shady Grove Fertility RSC, the Tinina Q. Cade Foundation, its vendors and their immediate family and/or household members are not eligible. Void where prohibited. All federal, state and local laws and regulations apply.

Drawing. On Friday, September 24th, 2010, 12:00pn EST a random drawing will be held to select one (1) winner from the names of all eligible entries. Winner does not have to be present to win. Prize must be claimed within ten (10) days by winner by signing and returning an Affidavit of Eligibility and Release. The Winner must commence the free IUI, IVF or Donor Egg Cycle within 6 months from the date of the drawing.

Prize. One (1) IUI, IVF or Donor Egg cycle to be performed at the offices of Shady Grove Fertility RSC(value: up to $10,000.00; Winner responsible for fees beyond $10,000). Surgical procedures must be performed at Shady Grove Fertility in Rockville, MD. All services, including the IUI, IVF or Donor Egg cycle, will be submitted to your insurance company. If payment is received from your insurance carrier on any of these services, SGFC will be entitled to keep these payments towards your services. Prize cannot be applied to the Shared Risk program.

Cycle includes the initial consultation, stimulations labs and ultrasounds, physician fees for retrieval and transfer, embryology lab charges, trial transfer and stimulation medications. Not included as part of the prize is medications beyond what is donated from EMD Serono, prescreening, any testing performed outside Shady Grove Fertility RSC, any treatment/surgery costs of side affects or complications, hospital charges, pesa/teas/tese/testicular mapping services and associated facility fees, urologist charges, social worker charges, ICSI, assisted hatching, microdissection, PGD biopsy, cryopreservation and storage fees, pregnancy blood work, ultrasounds and office visits, donor fees, donor prescreening or donor medications, or gestational carrier fees. This is a comprehensive list of exclusions at this time, there maybe additional tests/programs that may be considered exclusions, as well as any other test, products or services not specifically listed herein as part of the prize. Other restrictions may apply.

Prize is not refundable, or redeemable for cash, and no substitutions are permitted. Prize is transferable to one person. Prize transferee must have medical indication for fertility treatment to redeem prize. Prize may not be sold or exchanged. Federal, state and local taxes are the responsibility of the winner. In the event the winner cannot use the prize prior to the expiration date, the prize will be forfeited. Upon forfeiture, no compensation will be given. Current patients must have all monies owed to Shady Grove Fertility RSC paid prior to the start of the free cycle. Prize may not be applied to treatment in progress or past treatment. Odds of winning depend on the number of eligible entries received.

Publicity.The winner of the free treatment cycle is granting permission to share his or her story, name, and image with Shady Grove Fertility and the Cade Foundation for public relations. Failure to do so may result in forfeiture of the prize.

Liability. By participating in this drawing, entrants agree to these rules and decisions of Shady Grove Fertility RSC, which shall be final. Entrants release Shady Grove Fertility RSC and its officers, directors and employees from any claims, liability and/or loss resulting from participation in the drawing or the acceptance or use of the prize.
