1. What Insurance Companies does Shady Grove Fertility participate with?
   
   Cigna, Aetna, United, United Resource Network, MDIPA, Optimum Choice, OneNet (formerly Alliance PPO), Kaiser, Carefirst MD & DC, Blue Choice, PHCS, NCPPO, Anthem, Health Keepers, Coventry National Network (formerly First Health), Choice Care (Humana), Great West Life
   
   
   
2. Shady Grove Fertility does not participate with Coventry, Informed, Multiplan, MHIP, NPN, Tricare, and EHP.
   
   
   
3. What if Shady Grove does not participate with my insurance?
   
   Payment will be required at the time of service. Courtesy claims will be filed for PPO claims for you and the insurance company will pay you directly.
   
   
   
4. Will my cycle authorizations be transferred over to Shady Grove?
   
   Yes. Financial Services staff will transfer your authorizations for you.
   
   
   
5. Will the Financial Services staff remain the same or will I have to work with someone new?
   
   The Financial Services staff will remain the same for your benefit, authorization and referral questions. Shady Grove’s Central Business Office located in Rockville, Maryland will handle any statement billing questions you may have.
   
   
   
6. What Financial Programs does Shady Grove Offer patients with no insurance coverage?
   
   Shady Grove offers two different programs to choose from:
   1. Shared Risk Refund Program
   2. Self-Pay Global for IVF and IUI Treatments
   Both of these programs include anesthesia and operating/procedure room charges for IVF and IUI services. Please ask your Financial Coordinator about these two new programs.
   
   
   
7. Does Shady Grove offer Financial Programs such as Shared Risk and Self Pay Global for Donor Program?
   
   Yes.
   
   
   
8. Does Shady Grove work with Capital One?
   
   Yes.
   
   
   
9. How will the billing work?
   
   
   • Shady Grove Charges
     Shady Grove will bill for all physicians services (Drs. Katz & Yazigi), embryology services including embryo and sperm storage, andrology services, prescreening and cycle monitoring blood work and Embryo transfer and IUI procedure room charges.
     
     
   • GBMC Women’s Surgical Center Charges
     GBMC will charge for operating room fee for retrievals and any necessary surgeries. If you choose to participate in Shared Risk Refund Program or IVF/IUI Global treatment option operating room/procedure fee is already included for IVF and IUI Services.
     
     
   • Anesthesia Charges
     Anesthesia will be billed by Physicians Anesthesiologists Associates (PAA).
     
     If you choose to participate in Shared Risk Refund Program or IVF Global treatment option anesthesia is already included for your retrieval.
     
     
   • Urologist Charges
     If required, the Urologist will bill separately for PESA/TESE and GBMC Surgery Center will bill for operating room fee.
     
     
   • Additional Testing
     If additional testing, such as infectious disease blood work or genetic testing is required, an outside laboratory will bill you directly for these services. Please contact your lab for cost information.
     
     
   • Preimplantation Genetic Diagnosis
     Preimplantation Genetic Diagnosis lab charges will be billed by the Center for Preimplanation Genetic Diagnosis or another designated lab.
